There are two ways to secure your Shingles Vaccine:
1. ONLINE: Completing our online questionnaire is fast and easy!
- No driving to doctor's appointments and sitting in waiting rooms. No waiting on hold with your insurance company to determine coverage.
- ESI Medical will process your insurance information and determine your co-pay.
- Click below and fill out our pre-qualifying questionnaire and insurance information page.
- Once processed, we will contact and provide you with your insurance co-pay responsibility, and provide a prescription.
- Upon your approval, you will be contacted to set-up an appointment and confirm location to receive your vaccine. Payment will be accepted at the time of vaccination.
- A $15 prescription processing fee will be assessed only for those who utilize our site to secure the prescription and receive the vaccine.
2. TRADITIONAL METHOD:
- Set an appointment with your physician to receive a prescription.
- Fax prescription to 732-280-2872 or mail prescription to ESI Medical.
- Click the link below to finish online processing. No fees will be charged for processing.
- ESI will process your prescription and insurance information. We will contact you to confirm your costs and to order your dose.
- Upon your approval, you will be contacted to set-up an appointment and confirm location to receive your vaccine. Payment will be accepted at the time of vaccination.
ATTENTION: Merck, the pharmacy that manufactures the Shingles Vaccines, has announced a nationwide shortage. This has created longer than expected delays in shipment, which means some clinics may not be able to be held on their scheduled days. A representative from ESI Medical will contact you regarding any supply issues as we learn more.
